Notes from my playthrough
Was your paladin disarming traps in the final castle maze? Because that's where I noticed this as well. It also seems to happen with the wooden plank bridges right before the fight with the flying snakes. As for flank, it's an optional rule as per the 5E rules and for good reason. Advantage from flanking makes gaining advantage trivial, and makes many spells and abilities such as Faerie Fire and barbarian's reckless attack much weaker...

No rations, no adventure.
This may be a silly question but out of curiosity did your ranger have the Survival Skill? I suspect that may help in finding rations. At the same time, I agree rations not respawning in town or not spawning quickly enough is bad for exploration. The shopkeepers should be fixed so they have as much as you want to buy, in my opinion.
