Fighters and Extra attack at level 5
You get 1 Action Surge, which allows you to take another action. At level 5, fighters can make 2 attacks with a single action, so 2 actions results in 4 attacks.

How can I cancel a spell/buff
My only other idea is that the character who is flying isn't the same character who actually cast the Fly spell...? Otherwise, I guess submit a bug report and then either short rest or ritual cast spells to pass the time. If possibly, maybe first cast an AoE spell near the character who is supposed to be concentrating on Fly (for fun, include all characters in the blast) and see if that leads to Concentration...

Q about spell Thunderwave
Did you actually encounter the spell "Shatter" used by the enemy cleric? Shatter is a 2nd level ranged spell which also does thunder damage and allows Constitution Saving Throws for half damage similar to Thunderwave. It doesn't, however, push enemies back.

Cloak of Arachnida / Spider Climb is disappointing
+1 There's a few of these places throughout the game. I feel like spider climbing characters also can't get to a similarly-placed chest in the Library.

You should just be able to click the "concentrating" circle on the character's portrait/info section on the bottom left of the screen.
