Stuck in story progression at Cradle of Fire
I got some good advice in an early post but didn't understand what the writer meant. If you go to the transition door leading back to the mausolium but don't go back in the crypt area itself there is a third door in that dark room. On my screen it is so dark I never noticed the orc totem or the door. I wanderer around hitting alt in every room looking for anything that turned...

Stuck in story progression at Cradle of Fire
Tried a second run where I did the axe/key in a different order. Had the same problem happen. I have been to the Necropolis at least a dozen times now to see if he will spawn. I'll try the other surviving chief again. I know people have made it through here (and some complained about bugs at the main BBEG fight). If all else fails I will start from just before fighting the dark queen...

Stuck in story progression at Cradle of Fire
There are no unopened doors for me. I think the one you mean is the one to the Dark Lady part of the missions. My journal names a matching location on the map. Maybe I will just have to keep the axe and fight all the remaining orcs to advance. There is no way to leave the orc area to go do other things so I clear this mission or my game is stopped dead...
