I swapped my Paladin's Orc Waraxe with Javelins in the secon weapon slot then after throwing the Javelins when I went back into the inventory the Axe was gone

There are some severe inventory bugs in the game. It is not uncommon to have random items vanish from inventory when you drag an item in inventory.
