Holy retribution is bugged

When my cleric was shot with a bow this popup appeared 

It looks scarry and appeared after a non-melee attack.

Moreover, when I agreed, the character, in addition to the damage from the shot, also received damage from the ability. 

The character also caused damage to himself when he was attacked by Sorr-Akkath with poisonous spine

When did I use it as a reaction to a melee attack 

a) once the enemy received only damage from the ability 

b) and the second time from weapons and abilities
