Flaming Sphere states it hurts people that walk thru it.  It has yet to do this for me.

Yes, for some reason you have to use your bonus action to select the sphere and attack someone with it. Also it only deals damage when a character ends their turn inside the sphere

Came to report same issue.  Spell should function to enemies the same way it does to PCs (you shouldn't have an option to attack with the sphere, you can move it up to X tiles as a bonus action during your turn, anyone within 10 feet of the sphere rolls Dex save or takes 2d6 damage)

I also have had the situation where a conversation was triggered after a combat ended and one of the party members stood in the sphere and was dead after the conversation.

It worked the way it should for me in the Bone Keep, but nowhere else.

I set this spell at a chokepoint and half a dozen ghouls ran right through it. One even stopped in it. None took damage or even made a roll to avoid.

This happened to me at the fight right when your enter the catacombs are the first gem castle. Everything walked through the radius w/o saves or damage (friendlies and enemies), and as a bonus after the fight ended my paladin was still in the radius and instantly made 5 dex saves and took 19 damage.

The enemies need to be stupid enough to end their turn in the sphere's area of influence. Under certain conditions, one can force them to eat Attacks of Opportunity or end their turn in the sphere's influence.It's best for harassing ranged units, at least it provides it's own light.

The spell description needs to be updated to communicate this. I did notice when doing tests that roughly 1 in 10 times the sphere causes damage to units passing through it, but I could find no way to make this a predictable phonomenon and hence a useful one.
