Enemies attacking multiple times and still disengaging

Is anyone else seeing enemies attack 3 or more times and then leaving a threatened square without triggering an AOO?

You are certain the character that could launch the AOO hasn't already used their reaction for the round, either for AOO on a different enemy, or to protect an ally, or to cast shield, etc.?

Goblins and PC rogues can do this and it appears some vampire spawn learned the trick.

If they attack with multi-attack, meaning the 'monster ability' of multi-attack, then any bonus action ability to disengage would still be intact.
