Duel Wielder keeps losing his weapons

I've got an issue with one of my characters that is dual wielding.It seems that his main hand weapon keeps disappearing. The only way to get it back is to load an old save and hope it doesn't happen again. Is anyone else seeing this?

I kept dropping my offhand weapon, but it would be On the ground after the fight. But I think that was because I was accidentally throwing it, and this probably doesn’t apply to you.
