I created a couple characters, then decided I wanted to retool them, so I deleted them and recreated them with the exact same names, but different appearances. However, the in-game portrait created for them remained the old portrait, not a new one based on their new appearances. Verified that this follows them into the game proper.

Deleting the characters, exiting the game, and relaunching did get it to generate new portraits when recreating.
