Hello,
This is a minor bug but I witnessed it during two playthrough.
When fighting against the Remorhaz at the end of the quest Temple of the Lost Gods, it appear that the quest and the achievement linked to the fight validate as soon as only one of the enemies dies.
As I said, this is truly a minor bug, but I wanted to report it if it can help making Solasta one of the best RPG i've ever played!
